Things to Remember

It was packaged in the original box. It looked almost like a credit card, and you would have had to punch out the tool. If you do not have that, then use one of the suggestions posted in the answers as an alternative.

If the carrier has not activated the SIM card, then it will show that there is no SIM. After about 12 hours, take the phone to a storefront location for your device cellular service carrier. Ask for them to check out the phone to make sure all the cellular settings are correct. You might also want to connect the phone to the PC and use iTunes to see if you can activate the SIM card.

Sorry, this happened to you. All SIM are rather delicate. This is how they damage so easily if you drop your phone or force the card out of the SIM tray.
